Reporting a Claim?

New claims must be reported in writing by Fax, Email or Mail. 

If you are reporting a claim involving a vehicle insured by CHIC or CHIC/State National, see the instructions below:

1) If you were the driver of a vehicle insured by CHIC or CHIC/State National, complete this form. Include a police report if available. 

2) If you were the driver of a vehicle involved in an accident with a CHIC or CHIC/State National insured vehicle, complete this form. Include a police report if available. 

3) Return the form and police report by Fax, Email or Mail
